b) the proportion of those in breach who were transferred to an alternative provider on the NHS as a result?

The Trust does not hold this data: The Trust records transfers (national RTT code 20 and 21) but not the reason why they were transferred, i.e. because of a breach or if it was initiated by patients of trust

ii) Does the Trust record whether such transfers were initiated by patients or the relevant departments within the hospital? If so, what is the percentage split between the two?

See above answer i)a)

iii) As of November 2013, what is the longest waiting time for the following at the Trust?

a) any treatment/procedure;

64 weeks (November 2013): This wait could be because we were not allowing any pauses due to system issues, therefore this wait may include a proportion of time when the patient was not available

b) any diagnostic test

16 weeks (July 2013): Data gathered using the DM01 submission guidance
