Introduction

The responsibility of ensuring patient’s medication is suitable for use on admission lies with the nurses (see Appendix 3). Nurses are also responsible for checking and issuing the correct medication and supporting information to patients on discharge. This policy outlines the procedures that must be followed.

 

Scope

This guidance is relevant to: All Nursing staff, Pharmacists and Pharmacy technicians, in relation to the provision of medication and supporting information to patients on discharge.

 

Purpose

To ensure that all patients are discharged home with the correct medication and that appropriate information about their medicines has been communicated to the patient or a designated carer.
